Social worker turnover damaged understanding of case

A high turnover of social worker staff damaged a council’s understanding of a case where a 17-year-old fostered boy died, according to a Serious Case Review. The review of the case in Brighton and Hove, in which the boy hanged himself, concluded that his death could not have been predicted or prevented, but the difficulties in his having multiple changes of social worker were highlighted. The boy had complained about the changes in social worker, none of whom he had seen more than five times.
