A restructure of children’s services meant that ‘corners were cut’ and agencies failed to properly investigate the case of Child L, according to a serious case review by Somerset Safeguarding Children Board. The review found that there were missed opportunities to safeguard the girl and her younger sister, who were eventually taken into care. The upheaval going on at the time led to agencies failing to challenge social care decision-making, according to the review. It recommended a review of inter-agency procedures and strengthened guidance on the use of face-to-face meetings.
